Although I didn't have an epidural with my 3rd birth...really my 4th was my first completely natural birth. We do not (which is not why we decided to go this route). We payed a flat fee which was TINY in comparison to a hospital/doctor bill with no insurance. Plus my midwife gave us a deal for not having insurance and paying it all up front. Most insurances will cover a midwife (they also do well-woman visits) and other fees.
